Community Empowerment Project - Participate in the community-wide survey
(March 3, 2010) The Long Island Home: Broadlawn Manor (BLM), located in Amityville, has been involved in community empowerment, planning, coalition building and community organizing for decades. BLM has either lead or participated in collaborative efforts that sought to address the basic needs of our community members, as well as their physical and mental wellness, continued independence, and ongoing engagement in community activities. One focus of the New York State Office for the Aging is to ensure that our communities are “aging friendly”.
Through a community-wide survey, BLM would like to begin to identify the needs of those over the age of 55 living in the community, as well as any barriers to their full community participation and wellness. Once needs are identified, plans can be explored about how government and community groups can move forward to improve local residents’ abilities to live independently at home. We would like to hear from as many of our older residents as possible and have a goal of collecting at least 500 surveys. Caregivers of local residents are also eligible to complete the survey.
The survey we will be using, the Vital Communities Assessment, only takes a few minutes and is available to complete online at: www.zoomerang.com/Survey/?p=WEB22AAAFB42VU
Paper copies are also available and can be mailed to you at your request by calling Barbara Dunn at 631-608-5052 or by emailing Pat Morley at PMORLEY@longislandhome.org. We look forward to gathering your valuable input and thank you in advance for taking a few short minutes to help make the greater Amityville community an “aging friendly” great place to call home.
